Metabolic Assessment Form

Dr. Schroter now includes a metabolic assessment form with his new patient intake paperwork. The form is two pages and includes twenty categories. The questions asked in each category relate to different organs or systems in the body and Dr. Schroter is able to determine whether things appear normal or if there are areas of potential concern. It is the quickest and simplest way for Dr. Schroter to assess a patient’s health and make appropriate recommendations.
